Becoming a Member Your tax deductible gift helps to support the many library programs sponsored by FHLA. To enroll, fill out and mail in an enrollment card, available at the main counter at the library or in the Resale Bookstore, or simply write your name, address, the date, and the amount of your gift on a sheet of paper. You may also use the Membership Form link here. If you include your email address and phone number, we will be able to contact you about upcoming events. Mail your tax deductible check made payable to FHLA, to PO Box 17581, Fountain Hills, AZ 85269. If you prefer to sign up electronically, send your name, address, phone, email address, and date and amount of your gift to registration@fhla.net. You may also send your payment electronically, if you have online bill payment. Simply set up FHLA as a payee, using your phone number for the account number, and set up a one time payment for the membership level of your choice. |
